TMF’s programs and services—ranging from quality improvement consulting and technical assistance to research, data analysis and medical review—engage all aspects of health care and caregivers.

TMF Health Quality Institute is the national learning network contractor for the Comprehensive Primary Care (CPC) initiative of the Center for Medicare & Medicaid Innovation. On Nov. 4, 2015, the U.S. Dept. of Health and Human Services blog featured a CPC practice from rural Arkansas that is delivering high quality primary care partly through an expansion of its coordinated care services. TMF develops and delivers curriculum to guide primary practices as they broaden their services as they seek to lower health care costs, improve patient health and improve patients’ and families’ experience with health care services.
The Centers for Medicare & Medicaid Services (CMS) reports that Quality Improvement Organizations (QIOs) have helped to save nearly $1 billion by improving care transitions across the country.
