Christina Brown, MA, NLC, EMT, CPHQ, LSSBB
Christina currently serves as a subject matter expert for the Emergency Triage, Treat and Transport (ET3) team, providing implementation support to Model Participants across the nation. She also supports the Comprehensive Primary Care Plus (CPC+) team as a behavioral health integration expert and as a learning and diffusion lead for the Bundled Payment for Care Improvement (BPCI-A) initiative. Christina is an ambitious health care leader with more than 13 years of clinical and process improvement experience throughout federal, state and military health care settings with a focus on education and innovation.
Elisha Jewett, MPH, CPHQ
Elisha provides implementation and development support and serves as a subject matter expert for Center for Medicare and Medicaid Innovation Center (CMMI) programs including Comprehensive Primary Care Plus (CPC+) and Stakeholder Engagement and Awareness (SEA). Elisha is a skilled practice transformation leader and facilitator with many years of experience working with Colorado health care systems and practices. Elisha’s areas of expertise include data-driven improvement, care management, care coordination and patient experience. Elisha graduated with a Master of Public Health from the Colorado School of Public Health at University of Colorado Anschutz Medical Campus.
Kari Loken, CPHQ
Kari currently serves as a senior content expert for Comprehensive Primary Care Plus (CPC+), providing ongoing training and development for the practice facilitator team serving more than 950 primary care practices in five regions. She also assists in creating curricula and content for learning events. Kari is experienced in practice transformation and advanced primary care concepts, and assisted in the development of a practice facilitator training program. She was the primary trainer for her previous health care organization and co-authored content for the Agency for Healthcare Research & Quality’s "The Practice Facilitation Handbook".
Kari has more than 15 years of experience in practice transformation including coaching practices in quality improvement and training practice facilitators. She has experience in management for non-profit organizations and an independent physician association.
